PRINCIPAL DUTIES AND JOB
Responsibilities:
Conduct initial psychosocial assessment within the appropriate time frame after the patient's admission.
Coordinate and provide the delivery of social services in accordance with the interdisciplinary plan of care.
Continue assessment of patient/family psychosocial needs in accordance with the interdisciplinary plan of care to assist the patient/family in dealing with the psychosocial aspects of illness as evidenced by documentation.
Participate as an interdisciplinary team member.
Participate in case conference at least every two weeks.
Utilize community agencies and alternative resources to assist patients and their families in resolving unmet health care needs, financial and environmental difficulties.
Participate in orientation, in-service and staff meetings as scheduled.
Provide required documentation within established time frames.
Report pertinent observations to attending physician, Case Manager or Patient Care Coordinator.
Qualifications
What you will need:
Master's degree from a university or college accredited or approved by the Council on Social Work Education
Current Texas State Driver's License
Proof of liability insurance on file
Current Social Work license in the state of Texas
